Request rates come from server-side counters, so a failure the browser never got to report — offline, DNS, TLS — is not in them: treat each rate as a lower bound. Report counts are deduped distinct problems, not failure counts, so the two columns are different quantities and do not divide into each other.

Inspect the entire exact contributed photo. Reject anything containing personal documents, names, confidential/customer material, sensitive labels or serial numbers, or other content the contributor should not have shared. Do not assess mask quality at this stage. Acceptance permits controlled retention but leaves the sample structurally unavailable for model use.

Invoice workflow
The lines below are frozen on the order and are what the customer agreed to pay. Invoice them exactly as they read here — one line per insert, with the invoicing tool multiplying quantity by rate. The invoice is issued and collected outside this screen; the fields under it are internal tracking metadata and do not change what the customer owes.

Cancellation
Cancelling releases the customer's insert so they can edit it and order again. Nothing is deleted now; the artifacts are purged 90 days later. The reason is emailed to the customer word for word, so write what they need to change. An invoiced order cannot be cancelled here.
Approved inserts
Fabrication follows the manufacturing revision, never the newest one. There is no way to correct an order here: a design problem is cancelled with a reason, and the customer edits their own insert and orders again.
